Membership fees are based on age:

Membership may be paid monthly, quarterly, or annually. Patients age 30 and older receive a 15% discount for a spouse or second household member who also joins the practice. Because the practice intentionally maintains a limited patient panel, new membership requests may be placed on a waiting list if the practice reaches capacity. Membership fees are separate from medical services billed through insurance.
