  • Ages 12–29: $100 per month or $1,200 per year
  • Ages 30–44: $185 per month or $2,200 per year
  • Ages 45+: $250 per month or $3,000 per year

Membership can be paid monthly, quarterly, or annually. Patients age 30 and older also receive a 15% discount for a spouse or second household member who joins the practice.
